1. Arrear Details- Proforma GAIN PF-  As per Arrear G.O Details, Type Amount, Encashment date   on DA Arrears merged  up-31.03.2016 to be Excluded from 2015-16 Credit Card Balance in PF Loan Application Statement 

and Dearness allowance(DA)/ Pay Revision orders after 01.04.2016 Merged. One proforma should use for whole Schedule

2. Debit Details- Proforma GAIN PF-  

Details of Temperory Advance Loans  before 31.03.2016 

 (Refund after 01.04.2016- 

 and     Details of Loans after 01.04.2016 (Both TA and NRA)  To be written in the order of  Loan Encashment Date in the Proforma    

For TA Sanction Order Copy to be submitted with the Proforma
